I, {name}, being of sound mind and over the age of eighteen years, from this date forward, do hereby release BANG FITNESS LLC. Trainers/affiliates, Jerry St James Sano of all liability that may be incurred as a result of my participation in physical training at this facility located at 1681 North Zaragoza Rd. Suite B, 79936 El Paso, Texas, Texas. Being a responsible and aware adult I voluntarily choose to participate in physical training and its activities. I understand I should consult a physician before beginning any exercise programs. I am fully aware that participation in physical training includes strenuous exercise, weight lifting, boxing, Kickboxing, MMA and wrestling. I am aware that some of the participation is disagreeable, stressful, and necessitates mental control for me to successfully accomplish the goals I establish in physical training. I understand that I may postpone and /or terminate my participation in this physical training at any time. I understand that the use of these facilities is being provided in accordance with a membership contract that BANG FITNESS may postpone and / or terminate my participation in this physical training at any time. I understand that this facility is monitored by video surveillance 24 hours a day for the security and safety of the facility and those who use it. Any trespassers, vandals, and violators of the law will be prosecuted. I release the above individuals from any and all injuries that may be incurred by me from participation in physical training at BANG FITNESS. My aim in participating in physical training is to gain physical and mental control in a positive nature with no intention of developing strength and learning wrestling skills in malice toward any of my fellow man.

PHOTO RELEASE: I hereby authorize BANG FITNESS LLC to publish the photographs taken of me, and my name, for use in the BANG FITNESS LLC printed publications, social media and website. I acknowledge that since my participation in publications, social media and websites produced by BANG FITNESS LLC is voluntary, I will receive no financial compensation. I further agree that my participation in any publication, Facebook and website produced by BANG FITNESS LLC grants me no rights of ownership whatsoever. I release BANG FITNESS LLC its contractors and its employees from liability for any claims by me or any third party in connection with my participation.
